Role summary
This role involves providing holistic assessments and rehabilitation to patients in community beds who are recovering from hospital discharge. You will work as part of a multidisciplinary team to plan and develop goal-focused rehabilitation programmes for adults with complex physical and mental health needs. Job summary
The Clinical Beds team
provides holistic assessments to patients who require a period of
rehabilitation and assessment prior going home after being discharged from
hospital.
We work closely as a
multidisciplinary team alongside our partner services such as Adult social care
and continuing health care to aid patients and families make long term
decisions about their health and social care needs.
This is an excellent
opportunity to broaden your skill set in a holistic manner working with
occupational therapists and mental health nurses along with supporting junior
colleagues such as student nurses, assistant practitioners and rehabilitation
support workers.
It is anticipated that interviews will take place week commencing 26th October 2026
Main duties of the job
You
will need to demonstrate high standards of evidence based clinical practice,
have excellent interpersonal and communication skills and be able to prioritise
your workload, manage your time effectively, and work flexibly within the team.
You
will carry out profession specific, and holistic, assessments and evaluations
as required to promote independence and prevent unnecessary hospital
admissions. You will be part of the multi-disciplinary team; planning and
developing goal focused rehab
