Role summary
The Team Leader for the Children's Community Nursing Team (CCNT) provides strategic, clinical, educational, and operational leadership to ensure safe, effective, high-quality, child and family-centred care. Essential criteria
- RSCN/RN-Child with current registration
- ENB 998/997/mentorship module or equivalent
- Recognised Children's Community Qualification or equivalent experience

- Independent Prescribing or commitment to work towards
- Qualification in leadership
- Masters degree, working towards or equivalent experience
- Evidence of working with commisioners
- Experience of change management
- Comprehensive knowledge of children's community nursing practices and pathways
- Understanding of safeguarding children legislation, policies and procedures.
- Experience contributing to business cases and workforce planning

Job summary
The Team Leader for the Children's Community Nursing Team (CCNT) is responsible for providing strategic, clinical, educational and operational leadership to the service, ensuring the delivery of safe, effective, high-quality, child and family-centred care. The post holder will lead and manage the Children's Community Nursing Team, making effective use of resources to meet service demands whilst maintaining the highest standards of patient care and professional practice.
Main duties of the job
As a key member of the Children's Services senior leadership team, the post holder will support the implementation of national policy, local priorities and Trust objectives relating to children's community health services. They will ensure that all staff work within the professional standards, codes of conduct and scope of practice defined by the Nursing and Midwifery Council (NMC).
Working collaboratively with colleagues across primary, secondary and tertiary care, the post holder will lead the delivery of community-based services for children and young people with acute, complex and chronic health care needs. They will be responsible for overseeing the coordination of complex discharge planning processes, ensuring seamless transitions from hospital to home and promoting integrated, multidisciplinary care pathways.
