Role summary
This role is for an enthusiastic and motivated Healthcare Assistant to join the Chesterfield District Nursing Team. You will work within a community nursing team, visiting patients in their own homes and care settings. Responsibilities include assessing, implementing, and evaluating individualised patient care, as well as performing wound care, catheter care, and administering injections. The role requires excellent communication skills and the ability to travel within the North East Derbyshire locality.
Key responsibilities
- Assess, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
- Provide the delivery of high-quality care and actively support the District Nurse in the management of the health care environment.
- Work in close collaboration with the Integrated Community Teams.
- Carry out patient care including wound care, catheter care and administration of injections.
Essential criteria
- Good standard of general education including English Language & Maths GCSE at level C or above, or equivalent
- NVQ Level 3 in Health and Social Care or equivalent
- Excellent verbal, written and interpersonal skills
- IT skills
- Ability travel between care settings will be required
- Working in health and or social care environment
- Understanding of factors that influence health and wellbeing
- Understanding of the implications for, and responsibilities of the post for Risk Management and Safeguarding

- Abillity to problem solve
- Community experience
- MDT or integrated team working
- Palliative Care Experience
