Role summary
This role involves supporting patients in the community to live independently and improve their quality of life. You will work closely with a multi-disciplinary team, undertaking delegated clinical, therapy, and nursing-led activities. Key responsibilities
- Undertake a range of delegated clinical, therapy & nursing led activities to support service users.
- Provide safe, patient-centred and effective clinical activities in the community setting.
- Work within adults services to deliver a full range of healthcare activities.
- Be able and willing to travel throughout the service delivery geographical area.
Essential criteria
- NVQ Level 3 in Health & Social Care or willingness to work towards.
- Explain how you've worked without direct supervision, to achieve the objectives of a post.
- Tell us about your experience of direct patient care in a health or social care setting.
- Tell us about your experience of team working and an understanding of team dynamics including what factors make a team work well and what can go wrong and why.
- Able to use a variety of IT systems e.g. email, EMIS.

As a Community Support Worker, you'll play a vital role in our service. You will work closely with the multi-disciplinary team made up of clinicians such as Physiotherapists and Nurses to provide ongoing clinical activities such as, nursing tasks, rehabilitation, recovery and support with daily routines and personal care.
Successful candidates will have the support from their teams to be comfortable working independently in the community.
