Role summary
This role is for an experienced cardiac clinician to join a cardiac rehabilitation team. The practitioner will be the patient's first contact after hospital discharge, supporting their recovery and equipping them for long-term health. They will work with patients and the multidisciplinary team to address cardiovascular risk factors, provide education on heart disease, manage symptoms, and guide exercise. The role involves clinical assessment, caseload management, and leading outpatient programmes.
Key responsibilities
- Review patients on the ward to provide discharge information and discuss attendance at Cardiac Rehabilitation clinic and programme.
- Assess patients in outpatient clinics, considering physical and psychosocial needs, and providing essential post-discharge advice.
- Run telephone outpatient clinics and manage a caseload of patients, prioritising workload.
- Coordinate and lead an outpatient programme of 8 sessions, including cardiovascular exercise and health education.
- Maintain accurate patient records in accordance with data protection and liaise with multi-disciplinary team members.
- Keep up to date with advances in Cardiology/Cardiac Rehab and research, working within national guidelines.
Essential criteria
- Working towards post-registration qualifications/CPD in area of practice
- Relevant cardiac/cardiothoracic/critical care qualification at either level 2(diploma) or level 3 (degree) or equivalent experience
- Evidence of ongoing professional development through maintenance of professional portfolio and registration revalidation.
- Current ILS/ALS or willingness to undertake
- Driving license/able to travel between venues on daily basis
- Active professional registered with a professional body
- 4 years post registration including minimum of 2 years in cardiac care at band 5 or higher OR 2 years cardiac rehabilitation at band 5 or higher
- Recent experience working in a cardiac setting
- Involved in planning and supporting improvement and transformation, achieving cultural change to improve service quality
- Interest and understanding of cardiac rehabilitation environment.
- Experience of implementing local/specialist objectives
- Experience in foundational leadership and willingness to enact change management.
- Experience of management of the cardiac patient
- Advanced clinical assessment skills, assessment of symptoms, interpretation of clinical investigations and taking appropriate action in the risk stratification of patients post myocardial infarct/coronary revascularisation.
- Specialist knowledge of current cardiac care, management and secondary prevention
- Experience of involvement in research and audit at a local level within current or previous roles.
- Able to modify teaching and counselling skills to individual or group needs
- IT skills
- Very strong communication and interpersonal skills
- Professional approach to work and instil confidence
- Involved in supporting/management of support roles/students.
Desirable criteria
- Evidence of level 7 learning/qualification in relevant subject
- Prescribing
- Prepared to work towards independent prescribing if not yet achieved
- Proven leadership experience
- Level 4 specialist cardiac rehabilitation exercise instructor or equivalent
- Significant post-registration cardiac rehabilitation experience
- Experience of post-qualification work in a range of relevant settings treating cardiac patients

We are a fully certified service which meets the needs of over 600 patients annually who are recovering from a recent cardiac event. We can demonstrate excellent outcomes for our patients.
