Key responsibilities
- Act as a Named Nurse, maintaining accountability for the assessment, planning, implementation, and evaluation of patient care within a team nursing framework.
- Coordinate and facilitate timely, safe, and effective patient discharge planning in line with Trust policies and procedures.
- Deliver high-quality, patient-focused care, working collaboratively with colleagues across the multidisciplinary team.
- Support and educate patients, families, and carers to enhance understanding of health conditions, treatment plans, and recovery pathways.
- Maintain accurate, timely, and professional patient records and documentation.
- Uphold patient dignity, privacy, and confidentiality at all times.
Essential criteria
- Able to evidence thorough and up to date knowledge of nursing theory and practice (3)
- NMC Registration
- Understanding of the NMC Code of Practice and requirements (3)

- Acute UK hospital experience (3)
