Role summary
This role is an exciting opportunity for a Community Mental Health Nurse to join the Older Adult CMHT at Southport, Hartley Hospital. You will deliver high-quality, recovery-focused mental health interventions to older adults in North Sefton, including those with functional or organic mental illness. The position involves assessment, diagnosis, and treatment for dementia concerns, as well as supporting complex care packages and medication reviews. Regular travel between multiple sites is required.
Key responsibilities
- Deliver high quality, recovery focused mental health interventions to the population of North Sefton.
- Work with service users with a functional or organic mental illness.
- Provide brief therapeutic interventions with key working responsibilities.
- Support more complex packages of care to meet the needs of vulnerable clients.
- Complete routine reviews of memory medication to feed back to the MDT.
- Support more senior team members with duty roles.
Essential criteria
- Appropriate professional qualification or experience at Degree Level Registered Mental Health Nurse
- Experience of working with a wide range of people with mental health concerns.
- A demonstrated understanding of the human rights act.
- Be able to demonstrate sound knowledge base regarding: Effective care coordination, care planning and evaluation, quality and clinical audit, risk management and mental health act.
- Demonstrate a good knowledge of the NMC code of conduct and the nurse's responsibility to identify gaps in their knowledge base
- Demonstrate knowledge in the concepts of mentorship, preceptorship and supervision.
- Able to communicate effectively
- Possess good listening and negotiation skills
- Ability to work as a member of a team

Come and join us for an exciting opportunity in Older Adult CMHT at Southport, Hartley Hospital. We are working towards delivering a thoroughly modern mental health service that offers excellent standards of care and meaningful interventions to our service user population.
As a team we support older adults aged over 70 with a functional mental illness and we offer assessment, diagnosis and treatment to anyone with concerns about dementia.
This is a wonderful opportunity for someone who is interested in developing as a practitioner and person. Development opportunities will be available and regular clinical supervision and support will be provided.
PLEASE NOTE: This is a community based post which will required the post holder to travel regularly and in a timely manner between multiple sites in order to perform their duties.
