Role summary
This role is for an enthusiastic Occupational Therapist to join the Integrated Reablement and Assessment Service (ICRAS) in North Sefton. The service provides rapid response to prevent hospital admissions, facilitate early discharges, and deliver short-term rehabilitation and reablement in patients' homes and hub beds. The post holder will manage a caseload, undertake holistic assessments, devise treatment plans, and implement patient-centred interventions. A vehicle is required for travel across multiple sites in the Liverpool/South Sefton area.
Key responsibilities
- Undertake comprehensive holistic Occupational Therapy assessments and devise individualised treatment plans.
- Gain consent and engage patients throughout their therapy journey, enabling choice in their recovery.
- Plan and implement patient-centred individual and/or group interventions using graded activity to achieve therapeutic goals.
- Monitor, evaluate, and modify treatment to measure progress and ensure intervention effectiveness.
- Monitor and offer professional advice to team members regarding caseloads and working practices.
- Liaise with family members, carers, and other professionals during assessments and interventions.
Essential criteria
- Up to date knowledge and application of current best practice in relevant area of practice
- Application of therapeutic Outcome measures and Models of practice
- Application of the process of standard setting, audit, evaluation and review
- A sound knowledge of the core Occupational Therapy skills and their application in this field
- Be able to communicate effectively with service users within the specific client group
- Demonstrates a good knowledge of the Health and Care Profession Councils standards of Proficiency and the therapeutic responsibility to identify gaps in their knowledge base
- Demonstrate knowledge of the concepts of mentorship, and supervision
- Evidence of active participation in supervision and the appraisal process
- Experience of group and individual interventions with the specific client group
- Experience working in a collaborative way with the MDT and service users/carers.
- Good time management skills
- Apply and promote therapeutic skills, to include carrying out assessments, setting clear reablement goals and facilitating a wide variety of therapeutic interventions.
- Able to apply Person Centred Approaches
- Apply clinical skills and able to reflect on own and others practice.
- Excellent range of communication skills, able to motivate and support other staff.

ICRAS (Integrated Reablement and Assessment Service) is a supportive multidisciplinary service providing a rapid responsive approach to support admission avoidance and early hospital discharges and reablement.
We are looking for an enthusiastic and proactive Occupational Therapist to join our multidisciplinary teams in North Sefton. If you are motivated, with a passion to work within urgent care therapy to develop your clinical knowledge and experience within community care division we are looking for you and would love you to join our supportive team.
ICRAS Therapy teams work to support patients in their own homes, we provide a urgent care response service to prevent hospital admissions and facilitate early discharge and provide short term rehabilitation, and reablement services through delivery in our hub beds and within patients own homes.
Access to a vehicle is required for this role - being able to travel between multiple sites in the course of performing duties is essential. The role will involve regular travel across health care organisation sites in the Liverpool/South Sefton catchment area. Therefore, the post holder must be able to meet the travel requirements of this post.
