Role summary
This role is for a highly motivated Nurse Practitioner to join the Sunderland Community Frailty Team. You will be responsible for out-of-hospital care assessment and coordination for people with moderate to severe frailty, multiple co-morbidities, or complex needs. The role involves autonomous practice, comprehensive assessments, personalised care planning, and initiating investigations and treatments. You will work within a multidisciplinary team, focusing on enhancing self-care and improving the quality of life for older people.
Key responsibilities
- Coordinate the delivery and development of the specialist service using evidence-based practice.
- Be accountable for planning and delivering safe, effective, and compassionate patient care as an autonomous practitioner.
- Continuously improve clinical care standards in line with the clinical governance framework.
- Provide education and training to clinical staff across the Trust.
- Manage/lead practitioner-led clinics to review patients, optimize complex treatment regimes, facilitate hospital discharge, and prevent avoidable admissions.
- Undertake research relevant to the needs of the specialist area.
Essential criteria
- Awareness of relevant national health and social care policy.
- Knowledge of best practice in area of specialism.
- Thorough understanding of expanded roles/professional practice issues.
- Clinical teaching experience.
- Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required.
- Able to work without supervision.
- Competent in the use of IT packages.
- Proven interpersonal skills.
- Evidence of competency in relation to clinical practices appropriate to the post.
- Experience of the specialist which would normally be gained through working as a Band 5 or 6
- Experience of leading a team/supervising junior staff.
- Evidence of innovation in practice.
- Experience of coordinating care for a caseload of patients.
- Registered Nurse
- Degree in nursing
- Mentorship certificate/Practice Assessor training or demonstrable equivalent experience.
- Clinical skills accreditation or willingness to complete.
- Non Medical Prescriber or willingness to complete.
- Ability to assist patients to move as per Trust Moving and Handling Policy.
- Car Driver with ability to travel across sites.

Job summary
We are looking for a highly motivated Nurse
Practitioner to work within the Sunderland Community Frailty Team in
Sunderland. The team consists of Band 7 and Band 6 Nurse Practitioners who are
based in each of our six Community Integrated Teams. You will be responsible
for out of hospital care assessment and co-ordination of care and services to
people who live in their own home or nursing or residential care and are
identified as having moderate to severe frailty, multiple co-morbidities complex needs or who are identified as high intensity service users. You will work as a member of the
Multidisciplinary Team and will be required to attend and participate care home
and community MDTs. You will be an autonomous practitioner.
You will be responsible for pro-actively case finding patients with moderate
to severe frailty and undertaking CGAs Comprehensive assessment, personalised
care planning, providing support and advice to enhance self-care and improve
quality of life for older people and reducing level of frailty wherever
possible.
You will work autonomously, assessing individual
patient needs, initiating investigations, with support determining a
differential diagnosis and initiating appropriate holistic, evidence-based
treatment and care
