Role summary
This role is for a Social Prescriber and Recovery Worker to join the Autism and Learning Disability Liaison team. The successful candidate will support the implementation of care programmes under supervision, working across health, social care, education, and the voluntary sector. Essential criteria
- Higher Level Teaching Assistant or NVQ level 3 in Health and Social Care or equivalent experience relevant to the role.

- Undertaken further courses / training in Neurodivergence, Autism and / Learning Disability
- Has a positive, solution focused and progressive mindset to ensure the best outcomes for the child, young person and their family are achieved
- To be able to maintain patient records, both written and electronic
- Ability to recognise and understand presentations associated with Autism and / Learning disabilities
- Demonstrable ability in having difficult conversations and managing expectations
- Experienced in team working and working autonomously
- Examples of working using own initiative and problem solving
- Previous experience of working in a social prescribing role or similar
- Experience of working with Children and Young people in community settings, with Autism and / Learning disability
