Role summary
This role is for a Band 3 Rehabilitation Assistant to join the Workington ICC Community Rehabilitation team. The successful candidate will implement rehabilitation plans for patients in the community, providing direct care with a therapy focus, including supporting personal hygiene and daily living activities. The position requires working flexibly over 7 days, with shifts between 8:00 a.m. and 6:30 p.m. The team aims to facilitate early hospital discharge or prevent unnecessary admissions.

Desirable criteria
- Competency to complete clinical observations and basic diagnostics.
- Able to question and clarify information.
- Able to pass on knowledge to other team members.
- Care Certificate or the willingness and ability to undertake (applicable for all non-registered support workers that provide health care)
- Maths and English GCSE grade 4 (or equivalent) or the willingness to undertake the courses.
- Evidence of ongoing training and personal development.
- QCF level 2 or level 3 in Health, Social, Diagnostic and Therapeutic Support, or equivalent.
- Additional training in rehabilitation or caring for the older person.
- Knowledge and understanding of how an Integretated Care Community functions.
- Basic understanding of the Rehabiltation Assistant's role.
- Knowledge and understanding of the importance of rehabilitation and the promotion of independence.
- An understanding of how to support, with empathy and encouragement, individuals who may be unwell or have reduced functional capabilities.
- Self- awareness and appreciation of own limitations.
- Knowledge of personal care, function and mobility, nutrition, privacy and dignity.
- Understanding of reflective practice and its application.
- Knowledge of moving and handling policies and procedures.
- Knowledge of aids and equipment used to promote independence.
- Knowledge of the importance of risk assessment.

Job summary
An exciting opportunity has arose for a band 3 rehabilitation assistant to join Workington ICC. The successful candidate will be based with Workington Community Rehabilitation team.
We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation with older people, providing support to people who have long terms needs and who enjoys working in a team.
The Community Rehabilitation Team is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.
