We are looking for a compassionate individual to join our team as a Single Point of Contact Hub (SPOCH) Patient Navigator.
This role plays a key part in improving the experience of patients referred as urgent suspected cancer, or those newly diagnosed. You will support patients as they move through their diagnostic and treatment pathways, ensuring they feel informed, supported, and connected at every stage.
As a Patient Navigator, you will help streamline the patient journey by:
- Supporting patients with appointment management
- Ensuring timely access to diagnostics, MDT discussions, clinics, and surgery
- Signposting patients to clinical advice and appropriate support services
- Providing clear information on their pathway and next steps
- Managing administrative aspects of allocated cancer pathways
- Sharing clinical and investigatory information, including arranging required specimens (microbiology/laboratory)
- Making “good news” calls to patients discharged
