Role summary
This is an excellent opportunity for an NMC Registered Band 5 Triage Nurse to join the East Staffordshire Care Coordination Centre. Key responsibilities
- Provide remote triage of calls and referrals made to community nursing teams in East Staffs.
- Understand the scope of community nursing services and other MPFT/external services within each Primary Care Network to direct referrals appropriately.
- Occasionally deliver care to patients in a community setting to maintain up-to-date clinical experience.
- Maintain up-to-date knowledge of clinical aspects of community nursing to resolve clinical queries over the phone and enable effective remote assessment and triage.
- Have supervisory responsibilities in the absence of the triage lead nurse, including coordinating the team and allocating work.
Essential criteria
- Current NMC Registered Adult Nurse
- Experience of working as a Community Nurse.
- Ability to work autonomously and manage time and caseload effectively.
- Clinical knowledge relevant to the post.
Desirable criteria
- Training in Microsoft office software
- Evidence of post-registration training in areas related to community nursing (i.e. tissue viability; continence; palliative care; long term conditions management)
- Experience of working in a triage setting.
- Experience of carrying out remote (telephone) triage.

Job summary
We are delighted to offer an excellent opportunity for a NMC Registered Band 5 Triage Nurse to join our friendly and supportive East Staffordshire Care Coordination Centre in Burton.
We are looking for a passionate nurse who is innovative, motivated and enthusiastic with excellent communication and organisational skills to contribute to the delivery of a high-quality service within the community nursing environment.
You will contribute to the effectiveness and efficiencies of our community nursing teams by using your skills and nursing experience to support the team with triage of calls and referrals made to the community nursing teams. You will actively liaise with patients; carers and service providers to support clinical decision making and make appropriate decisions following comprehensive remote triage (using available information from referral information and electronic clinical systems) to ensure the safe management, in response to calls and referrals received by the community nursing service.
You are activity encouraged to develop skills via our CPD programmes, and you will be well supported by your peers and team leaders to do this.
