Role summary
This role is for a motivated and enthusiastic Community Mental Health Practitioner within Northamptonshire Healthcare NHS Foundation Trust's Adult Community Mental Health Services, specifically in the Kettering & Corby Area. You will provide planned and effective mental health support and interventions, working collaboratively within a multi-disciplinary team. Key responsibilities
- Provide planned and effective mental health support and interventions tailored to individual needs.
- Work in partnership with service users, their loved ones, and other services to ensure holistic care.
- Collaborate closely with other agencies and organisations to promote health and wellbeing.
- Act as a keyworker, providing a main point of contact and continuity of care for service users and carers.
- Contribute to a collaborative, recovery-focused, and trauma-informed multi-disciplinary team approach.
Essential criteria
- Professional Registration (NMC/HCPC)
- Post-registration experience in a community mental health setting
- Experience managing a defined caseload of patients with complex needs
- Risk assessment and risk management (e.g. suicide, self-harm, safeguarding)
- Experience of care planning and care coordination (CPA or equivalent)
- Ability to work autonomously and make clinical decisions
- Ability to manage workload and prioritise effectively
Desirable criteria
- Facilitating groups/ workshops to patients, carers and or professionals Staff management experience.
- Knowledge of mental health conditions and treatment pathways
- Experience working within MDT environments
- Experience supporting junior staff / students
- Experience supervising staff
