Role summary
This role is for a Band 5 Rapid Response Nurse to join a community team in North Lancs on a fixed-term contract for up to 12 months. The primary goal is to deliver urgent community response, providing a 2-hour crisis response to acutely unwell patients to prevent hospital admissions. The nurse will undertake holistic assessments, develop care plans, and work collaboratively within a multidisciplinary team.
Key responsibilities
- Support the delivery of the urgent community response standard.
- Provide a 2-hour crisis response to acutely unwell patients in the local community.
- Work within established Virtual Wards.
- Undertake holistic assessments in complex and challenging situations.
- Develop and deliver robust programmes of care for patients and carers.

Job summary
North Lancs Rapid Response are looking for a Band 5 Nurse to join our team in the community to cover a period of maternity leave on a fixed term contract for up to 12 months.
We are seeking a hardworking, dynamic, proactive, individual to join our well established multi disciplinary team.
You will support the delivery of the urgent community response standard which aims to provide a 2-hour crisis response to patients who are acutely unwell within our local community to avoid unnecessary hospital admissions. As well as working within the established Virtual Wards.
This is can be a challenging role , every day is different but exciting and rewarding.
