Role summary
This role involves working as a Clinical Dementia Advisor within a multidisciplinary Community Mental Health Team (CMHT). The post holder will provide clinical advice and interventions for patients with a formal dementia diagnosis, focusing on moderate needs to prevent community breakdown. Key responsibilities include leading care reviews, planning, and implementing evidence-based care for patients and their carers. The advisor will also offer professional advice to other agencies and act as a role model for junior staff.
Key responsibilities
- Be responsible for the further assessment, planning, implementation and evaluation of evidence-based care for patients diagnosed with Dementia.
- Conduct appropriate clinical risk assessments for patients contacting the team.
- Provide effective advice, guidance, and decision-making, coordinating reviewed care plans within MDT support.
- Promote carer and patient involvement within the service and its provision.
- Work alongside a Dementia advisor from the Alzheimer's society within a tiered model of care as part of the larger MDT.
- Ensure carer's assessments, education, and support are delivered according to NICE guidance and clinical pathways.
Essential criteria
- RMN, RGN
- Significant experience of working with people with a diagnosis of dementia
- Understanding of the needs of people affect by Dementia.
- Sound working knowledge base of the different subtypes of Dementia and their presentation.
- Working and supporting people with Dementia in the community as part of an MDT.
- Previous experience using supportive intervention to maintain a person with Dementia to remain in the community
- Access third partner agency input for support.
- Carer support and education.
- Demonstrates advanced, comprehensive communication skills.
- Clear/concise command of English written/oral.
- Ability to formulate plans for service strategy.
- Coordination of multiprofessional team activities
- Ability to formulate policies.
- Comfortable with basic IT skills
Desirable criteria
- Commitment to further education i.e., up to degree if not already qualified to that level. Mentoring or equivalent training qualification Willing to undertake the course.
- Willingness to participate in associated research and development.

Job summary
The post holder will work as a member of a multidisciplinary CMHT, the post will be for those patients that have already been assessed and given a formal diagnosis of Dementia by the team.
The model for the post diagnostic care is a tiered step up and step-down model of care in partnership with the Alzheimer's society led by Rdash. This post is for the moderate level of patient need that requires clinical advice and interventions with multiple complexity of need to prevent break down in the community.
To be the lead professional for the review and coordination of care needs, planning and implementation of evidence-based care to this defined patient group and their carers as part of a multidisciplinary approach.
