Role summary
This role involves conducting holistic mental health assessments for individuals with complex and severe mental health needs. You will work as part of a fast-paced, diverse team, collaborating with GPs and other community mental health services. The position requires independent work, strong communication skills, and frequent travel within Cornwall. It's an opportunity to contribute to a developing assessment service and help shape new ways of working.
Key responsibilities
- Manage referrals to secondary mental health services via triage and initial assessment.
- Work in partnership with GPs, other referring agencies, and community mental health teams.
- Conduct holistic mental health assessments for individuals with complex and severe mental health needs.
- Travel frequently to various locations throughout the county to perform duties.
Essential criteria
- Relevant professional registration - RMN, OT, SW, RNLD
- Evidence of post basic training
- Current professional registration
- To have a Sign Off Mentor (SOM) qualification as in accordance with the NMC and be able to sign off students as proficient of registration.
- Registered mental health professional with substantial post registration experience.
- Previous experience working in a community setting
- Education and Qualifications
