Role summary
This role involves supporting patients and their carers diagnosed with renal stones throughout their treatment journey. The post holder will provide direct support, advice, and signposting, and assist with holistic needs assessments. They will also undertake administrative and clinical tasks as delegated by Clinical Nurse Specialists, working as part of the Urology multidisciplinary team. The role focuses on ensuring a smooth patient pathway and managing non-complex workload.
Key responsibilities
- Highlight potential delays in patient pathways and work to mitigate their impact.
- Support Urology CNS's by managing non-complex workload and triaging calls.
- Liaise and refer to other healthcare professionals as appropriate.
- Coordinate patient pathways by managing information and collaborating with various departments.
- Assist in developing end-of-treatment summaries for primary care communication.
- Ensure timely booking of outpatient appointments.
Essential criteria
- oNVQ level 4 or level 3 plus additional training to diploma level or equivalent
- oSubstantial clinical experience.
- oExperience of working within multi-professional teams.
- oExperience in use of data management
- oExperience of coordinating patient workload.
- oExperience of coaching/teaching patients and their carers
- oBe an effective communicator both written and verbal
- oBe able to work as part of a multi professional team
- oDemonstrate good interpersonal skills
- oAble to use information technology
- oUnderstanding of patient centred care
- oMust be able to demonstrate behaviours consistent with the Trust's values. (As detailed in UHCW's Values in Action document below)

Job summary
The Stone pathway Navigator will form an integral part of the multidisciplinary team in caring for the physical, social and psychological needs of patients and carers diagnosed renal stones throughout their treatment pathway.
The post holder will have daily direct contact with patients and their families, providing support, advice, signposting and assistance with the management of renal stones. This will include the arranging and triaging of the holistic needs assessments as part of their core role.
The role will also involve the undertaking of administrative and clinical tasks deemed suitable to the role by the Clinical Nurse Specialists (CNS's).
The post holder will work as part of the Urology multidisciplinary team under the guidance and supervision of a registered practitioner.
