Role summary
This role is for a Community Health Care Support Worker within the Urgent Community Response team in Hillingdon. The successful candidate will support registered nurses and allied healthcare professionals in delivering high-quality, patient-centred care and rehabilitation to patients in their own homes. Duties include performing clinical skills like venepuncture and wound care, working on a rota system including weekends, and building relationships with patients and their families. Key responsibilities
- Support registered nurses and allied healthcare professionals in delivering high quality, clinical care and rehabilitation with patients in their own homes.
- Perform clinical skills such as venepuncture, wound care management, and catheterisations.
- Provide care calls in the community and assist with basic life support.
- Work effectively as a valued member of the community team, including on a rota system (08.00hrs - 22.00hrs) including weekends.
- Build good relationships with patients, their families and carers, and work in partnership with the multi-disciplinary Integrated Care Team.
- Travel around the borough of Hillingdon to visit various locations.
Essential criteria
- NVQ 2/3 or equivalent
- GCSE English and Mathematics level C or above
- Previous experience of working with a multidisciplinary team
- Experience of providing care in a clinic setting
- Experience of working with older people
- Excellent time management skills, with the ability to work effectively under pressure, meet deadlines and prioritise workload
- Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team
- Ability to communicate with members of the public and health and care providers
- Able to communicate with people who are in pain or distress with calm and compassion
- Ability to move between sites working across health and social care as required by the needs of the development programme
- Be able to identify risks to patients and seek guidance from Team Leader/ Deputy Team Leader
- Ability to work on own initiative
- Ability to take part in reflective practice and clinical supervision activities
- Good inter-personal skills
- Ability to understand and implement the Equal Opportunities Policy at a level appropriate to the job.
Desirable criteria
- Foundation degree in nursing or equivalent
- Experience of working with people with dementia or other cognitive impairments
- Experience of supervision in practice as a supervisee
