Role summary
This is a part-time Band 6 Mental Health Practitioner role within a Community Mental Health Team for older adults. The practitioner will manage a caseload, delivering clinical skills and therapeutic interventions, and act as a care co-ordinator. The role involves significant travel around Cambridgeshire and Peterborough, requiring a full UK driving licence and access to a vehicle. It offers an opportunity to join a supportive multidisciplinary team with CPD opportunities.
Key responsibilities
- Act as a care co-ordinator for service users and promote carer involvement.
- Manage a caseload, delivering diverse clinical skills and building therapeutic relationships.
- Empower service users in their recovery journey towards effective discharge.
- Provide assessment and interventions for people with significant mental health conditions.
- Offer training, advice and guidance to colleagues on common mental health conditions.
- Participate in MDT meetings and ensure key information is recorded in shared care plans.
Essential criteria
- Relevant professional registered qualification (Registered mental health nurse, Social Worker or Occupational therapist)
- Training in supervision and appraisal
- Significant relevant experience in working within a mental health service
- Experience of working with older people with mental health needs
- Experience of clinical and managerial supervision of junior staff
- Able to demonstrate commitment to high quality care and service provision
- Excellent clinical skill in assessment and interventions for common mental health conditions
- Excellent communication skills and ability to summarise information and communicate with colleagues
- Excellent understanding of statutory framework and care commissioning including: safeguarding, commissioning of care and Mental Health Act and Mental Capacity act
- Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.
- Ability to sit for sustained periods of time
Desirable criteria
- Post qualifying training
- Non-medical prescribing qualification
- Experience of working with an older people's community mental health service.
- Have a lived experience of mental health challenges
- Experience of working with nursing home and care providers
- Experience as safeguarding lead

We are currently looking to recruit a full time Band 6 Mental Health Practitioner (Mental Health Nurse, Occupational Therapist or Social Worker) to join our small and supportive Community Mental Health Team for older adults, embedded within an exciting Integrated Care model. This is a great opportunity to join our friendly and dynamic multidisciplinary team, with lots of support and CPD opportunities. There is free parking onsite.
We provide patient focused, evidence-based mental health interventions and assessment for people aged 65 and above, with a range of Functional and Organic mental health illness. You will manage a caseload of service users, delivering a range of clinical skills and therapeutic and recovery-based interventions. The work is varied and rewarding.
Your primary area of coverage will be Central Cambridge , but you must also remain flexible to support other parts of Cambridge as needed. Regular travel will be essential for this role therefore, successful applicants will need to have a drivers License and access to a vehicle for work.
The post is based at Brookfields Hospital. CPFT supports agile working, with flexibility for work-life balance. Visits to the team are welcome.
