Role summary
This role is for an enthusiastic Health Care Assistant to provide safe and effective care to patients in their own homes as part of a Community Nursing Service. The successful candidate will deliver high-quality personal and palliative care, work independently, and manage clinical supplies. They will ensure adherence to Trust policies, maintain confidentiality, and contribute to a supportive care environment.
Key responsibilities
- Deliver high-quality care to patients in their own homes in accordance with individual care plans.
- Provide personal and palliative care, ensuring dignity, respect, and compassion.
- Work independently and autonomously within the community setting.
- Manage the ordering and organisation of clinical supplies and stationery.
- Adhere to Trust policies, procedures, professional standards, health and safety regulations, and infection control guidelines.
- Maintain patient confidentiality and handle information sensitively and responsibly.
Essential criteria
- Excellent communication skills verbal and written
- Wound care and pressure ulcer knowledge/experience
- Ability to provide personal care to patients in the terminal phase of life
- Ability to undertake clinical intervention for care of patients as directed by Registered Nurses
- Ability to work well within a team
- Evidence of time management skills and ability to prioritise.

- GCE/GCSEs Grade A*- C/level 9- 4 in Maths and English or Equivalent or Willingness to undertake Level 2 Functional Skills Qualifications in Maths and English
- NVQ Level III in Healthcare/equivalent qualification
- Demonstrable experience of working in a healthcare setting
- The ability to travel independently across the Trust
- The ability to work flexibly in accordance with service needs

- Use of SystmOne/EMIS
- Care certificate
- Community healthcare experience
