Role summary

This role involves delivering expert primary healthcare services to adults within HMP Aylesbury, focusing on physical health. The post holder will conduct assessments, run clinics, and provide a range of health treatments and interventions. They will offer clinical guidance to staff on physical health and wellbeing, ensuring high standards of care in a changing healthcare environment. The role requires promoting professionalism within the nursing team and contributing to service development.

Key responsibilities

  • Plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence-based models.
  • Set up processes/protocols for effective assessment, planning, implementation and evaluation of care for patients with physical and mental health and substance misuse problems.
  • Safeguard vulnerable patients in line with Trust safeguarding policy and apply local Risk Assessment and management protocols.
  • Make appropriate referrals to other available services for longer term interventions and co-ordinate multi-disciplinary clinical practice.
  • Liaise with other professional services and work collaboratively with staff of other disciplines (e.g., officers and governors) to provide integrated and coordinated care.
  • Maintain the highest standards of clinical record keeping, including electronic data entry and recording, and ensure diversity issues are sensitively considered.

Essential criteria

  • oWillingness to undertake further training relevant to clinical area.
  • oKnowledge of medication rounds and the safe storage and administration of drugs.
  • oKnowledge of, and ability to follow, NMC code of conduct
  • oDemonstrates the ability to manage work under pressure
  • oAbility to assess plans, implement and evaluate patient care & act accordingly if untoward changes occurred.
  • oKnowledge of risk assessment and management and its implementation.
  • oAbility to treat patients with respect and dignity.
  • oAbility to use computer to access patient information and use email.
  • oAble to use computer software packages such as MS Word and Excel
  • oSensitive to colleagues and able to promote a positive motivating environment.
  • oProblem solving skills & Stress management skills & Conflict resolution skills
  • oAbility to maintain patient records within standards required by locally, Trust & NMC
  • oUnderstand the different roles of the multi -disciplinary team and own role within it
  • oAbility to demonstrate evidence -based practice & understanding of clinical governance in maintaining quality of care
  • oAbility to provide and accept feedback, support, clinical/managerial supervision
  • oKnowledge of current developments in specialist clinical area.
  • oExperience at senior band 5 grade
  • oExperience in providing and receiving supervision
  • oExperience of running clinics and/or group work.
  • o Ability to keep accurate records.
  • oPractical experience of providing high quality care and exceeding expectations
  • oRegistered General Nurse
  • oEvidence of personal and professional development in Nursing since Registration

  • oExperience of working within a secure environment
  • oExperience of systmone IT system
  • oPost registration qualification appropriate to speciality
  • oENB 998 or equivalent

Job summary

The post holder will deliver an expert healthcare service to adults within HMP Aylesbury. They will offer advice to clinicians, allied healthcare professionals.

They will provide a comprehensive primary care physical health service and work primarily within primary care which is part of an integrated healthcare service which includes Primary care, wellbeing (which includes mental health and substance misuse).

To undertake physical health assessments, undertake clinics, provide and evaluate a range of health treatments and interventions, to meet the patient's healthcare needs and contribute to the wider continuous development of healthcare services.

Demonstrate knowledge, skills and understanding of health issues and treatment options to provide physical health clinical guidance to all staff on matters relating to physical health and wellbeing.

To ensure high standards of primary care services are delivered in a changing healthcare environment, ensuring that the needs of the patients are at the centre of practice.

The post holder is expected to continually promote high standards of service within the nursing team and contribute to development of a culture of professionalism within the team.
