Role summary
This role is for a caring and compassionate Community Mental Health Practitioner to deliver mental health care and treatment within the Enhanced MH Team West in Barnsley. The successful candidate will manage a caseload of approximately 25 people, coordinating their care, liaising with other professionals, and managing care plans and risk assessments. The team operates using the Flexible Assertive Community Treatment (FACT) model, providing high-quality face-to-face care. The role involves flexible working hours and participation in a daily shift coordinator rota.
Essential criteria

- Good communication skills. Ability to work on own initiative, with reference to policies and procedures.
- A current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).

08.30 - 19.00 Mon to Fri & 9-5 on weekends & BH. We are family friendly
