Key responsibilities
- Proactively work with services across boundaries to support admission avoidance/prevention and facilitated discharge.
- Champion a personalised case management approach, accessing multiple services to ensure comprehensive care for health and wellbeing.
- Gather information early and present to clinicians to aid decisions and prevent unnecessary hospital admissions.
- Promote access to relevant services and cascade information to support patient discharge, following up in the community.
- Link patients directly into the Co-ordinated Care Pathway, engaging relevant health, social, and VCSE services.
- Provide patient choice and ensure access to appropriate services, monitoring to reduce deterioration risk and prevent hospital admissions.
Essential criteria
- A-level/ NVQ 3 or equivalent experience in admin / business / marketing / customer service environment
- Experience and confidence working with a range of IT solutions
- Experience of working with patients/service users in a clinical environment
- Proven experience of working to deadlines.
- Problem solving abilities.
- Experience of working as part of a multidisciplinary team.
- Experience of dealing with sensitive/confidential information
- Proven administrative experience
- Leadership & Development qualities

- Degree level education

Job summary
The Care Navigator service in Manchester & Trafford is part of the Integrated Neighbourhood Teams. The service offers a community facing service with focus on delivering a seamless interface between hospital and community services, ensuring the people of Manchester have access to the right services at the right time in the right location.
The Care Navigator service consists of non-clinical staff members, who work closely alongside existing services across hospital and community settings including health, social care and Voluntary, Community and Social Enterprise (VCSE) organisations. The service is managed and supported by the Integrated Neighbourhood Lead as part of the Integrated Neighbourhood Teams in Manchester. The Trafford Care Navigator service is part of the wider team and are managed by the Specialty Lead directly.
The Care Navigators link directly with community based services and are able to signpost and support people to access services in the community. Providing the patient has given permission to be discussed by the Neighbourhood team across health, social care and VCSE organisations, neighbourhood teams are able to identify patients for discussion by getting in touch with a care navigator for that relevant neighbourhood.
