Role summary

This role is for a Senior Staff Nurse in the Unplanned Care Triage Hub, focusing on quick responses to patients at home to assess, plan, manage, and evaluate issues, prevent hospital admissions, and facilitate early discharges. The successful candidate will support the clinical band-7 with daily leadership and management of the team, liaising with the wider multi-disciplinary team. It requires excellent clinical skills, leadership qualities, and a commitment to high standards of patient care. The role involves providing triage assessment, clinical support, and professional expertise to patients, carers, and healthcare professionals accessing the Community Nursing Service.

Essential criteria

  • RGN
  • Current NMC registration
  • Community Nursing experience, minimum 12 months

Desirable criteria

  • V100/V300 prescribing for nursing or equivalent.
  • Ability to work collaboratively with ither services and make detailed onward referrals.
  • Demonstrate a range of clinical skills
  • Understanding and /or experience of managing patients with long-term conditions
  • Knowledge and Skills
  • Knowledge of the of the following areas - Leg-ulcer management, end-of-life care, diabetes, compression-bandaging and wound-management, referrals to other agencies.
  • An awareness of current Health and Social Care Policies.
  • Demonstrate an understanding of the role of the District Nursing service
  • Able to communicate effectively in challenging day-to-day situations.

A vacancy has arisen within the Unplanned Care Triage Hub, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team; and liaise with the wider multi-disciplinary team.

The successful candidate will have excellent clinical skills, demonstrate leadership qualities, implement high standards of patient care, and ensure a positive patient experience.

The Senior Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to patients, carers and health and social care professionals accessing the Community Nursing Service.

Senior staff nurses will have a minimum of 12-months' community nursing experience.
