Role summary
The Shrewsbury and Telford Hospital NHS Trust is recruiting a Trainee Critical Care Outreach Practitioner to join their expanding 24/7 Critical Care Outreach Team. This role involves providing early recognition, assessment, and management of deteriorating patients across the Trust. You will deliver patient-centred care, assess and manage critically ill patients, and provide autonomous specialist advice. The role includes follow-up assessments, participation in the cardiac arrest team, and clinical leadership. Successful completion of the training programme leads to a substantive Band 7 Critical Care Outreach Nurse Practitioner post.
Key responsibilities
- Work autonomously within all specialities across the Trust, providing expert clinical practice and leadership on a 24-hour shift rotation.
- Perform comprehensive patient assessments based on history taking and physical examination of acutely unwell adult patients.
- Assist and facilitate timely admission to critical care areas and involve the multidisciplinary team in decision-making.
- Respond to cardio-respiratory arrest and medical emergency calls, leading in emergency situations and debriefing teams.
- Enable timely discharges for patients from the Critical Care Unit by supporting their continuing recovery and rehabilitation.
- Act as an independent non-medical prescriber.
Essential criteria
- Registered General Nurse with active NMC registration
- Critical Care Course
- GCSE Grade 9-4 (A*-C) or equivalent in Maths and English Language
- Ability to work/study at Master's level
- Proven post registration experience within a Critical Care setting at Band 6 Level or above (or equivalent)
- Proven experience of working within a Critical Care Outreach or equivalent service
- Experience of autonomous decision making
- Evidence of extensive professional/clinical knowledge in Critical Care supplemented by specialist clinical training and CPD
- Proven experience of management of deteriorating patients and acutely unwell patients
- Knowledge of role and responsibilities of the post

Job summary
The Shrewsbury and Telford Hospital NHS Trust is recruiting a Trainee Critical Care Outreach Practitioner (CCOP) to join the Critical Care Outreach Team (CCOT) during the expansion to a 24/7 Trust-wide service. This role is ideal for a compassionate, motivated individual committed to improving outcomes for acutely deteriorating patients.
The CCOT provides early recognition, assessment, and management of deteriorating patients across the Trust. As a Trainee CCOP, you will deliver patient-centred care, assess and manage critically ill patients, provide autonomous specialist advice, and work collaboratively with the multidisciplinary team to support escalation decisions and facilitate timely admission to critical care.
The role includes follow-up assessments of patients discharged from critical care, participation in the cardiac arrest team, and providing clinical leadership and support in the day-to-day operation of the outreach service.
