Role summary
As a Band 6 Occupational Therapist, you will support the Harlow Integrated Community Care Team (ICCT) in delivering proactive, neighbourhood-based care to help individuals remain independent at home. You will provide specialist occupational therapy assessment and intervention for those with complex physical health needs, aiming to prevent hospital admissions and facilitate timely discharge. Working within a multidisciplinary team, you will develop person-centred care plans and collaborate with various community services to promote independence and wellbeing. The role focuses on integrated working, early intervention, and population health approaches.
Key responsibilities
- Deliver high quality, skilled clinical care to patients and provide rehabilitation and long term care/conditions management in a variety of community settings.
- Manage a caseload of patients with complex health & social care needs, using evidence based, client-centered principles to assess, plan, implement & evaluate interventions.
- Utilise specialist skills and knowledge to undertake comprehensive assessments of patients and analyse information to formulate accurate interventions.
- Collaboratively discuss and agree SMART goals and outcomes for treatment with patients, ensuring understanding and consent.
- Evaluate patient progress, reassess and review treatment programmes as required, determining potential for rehabilitation and future care needs.
- Plan and prioritise own workload, delegate tasks, facilitate safe patient transfers, assess and clinically justify equipment provision, and identify/undertake risk assessments.
Essential criteria
- Degree or equivalent in Occupational therapy
- Relevant training appropriate to role
- Broad range of experience across a sample of clinical areas
- Evidence of wide range of clinical and non clinical experience
- Knowledge of NHS polices and Neighbourhood and proactive care, preventing admissions and enabling people to remain independent at home.
- Experience of rehabilitation and functional assessment
- Experience of working with adults with physical health needs
- UK licence
