Role summary
This role involves supporting the District Nurse Team Leader with leadership initiatives, including caseload and staff management within the District Nurse Service. The post holder will provide nursing care through home visits and manage complex patient cases. You will manage a clinical caseload and work weekends on a rotational basis. The position offers opportunities to develop clinical skills and leadership styles.
Essential criteria
- Currently registered NMC Adult Nurse
- Evidence of relevant Continuous Professional Development
- Numeracy skills required for the safe administration of medicines

- Mentorship qualification
- Previous experience as a Band 5/6 community nurse or equivalent level
- HR skills training e.g. recruitment, managing absence
- Care planning: being able to assess patients, develop, implement and evaluate a care plan
- Empowering and working in partnership
- Knowledge and understanding of Safeguarding/Advocacy
- Knowledge and understanding of Mental Capacity Act and Deprivation of Liberty Safeguards
- Knowledge and understanding of Mental Health Act
- Manual Handling patients

- Able to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary
- Able to maintain electronic patient records
- Clinical Skills Specific to Service Catheterisation - Wound care - Palliative Care & End of life care - IV administration - Injections - Doppler assessments - Compression bandaging - Diabetes Management - Venepuncture
- Coordinate work allocation and shifts (if appropriate)
- Work well with other members of the team including communicating effectively, being kind and considerate, reliable and enthusiastic
- Manage others and contribute to the management of the team

Job summary
The successful candidate will be required to support the DN Team Leader with leadership initiatives including caseload and staff management within the District Nurse Service structure. Depending on the demand of service, the post holder will be expected to support with any new initiatives including post Covid business as usual . You will also be required to complete home visits to provide nursing care, as well as providing complex patient management and support.
You will be expected to manage a clinical caseload which will meet the needs of all patients across the local landscape and network. The post will also allow unlimited opportunities to develop clinical skills and test your leadership styles accordingly.
The role requires the post holder to work weekends on a rotational basis to support and coordinate weekend working across the locality.
Candidates will need to have a Full UK driving licence and access to a car .
