Role summary
This is an exciting opportunity for a qualified Nursing Associate to join the Rapid Response Team within Adult Physical Community Health Services. The role involves providing urgent responses to patients in health or social care crises, including face-to-face responses, falls pick-up, and virtual ward elements. The post holder will deliver individual plans of care, participate in complex care delivery, and maintain professional record-keeping. This role requires a committed, motivated individual with excellent communication skills to work autonomously and as part of a supportive team.
Key responsibilities
- Accept delegated assessment responsibility and deliver individual plans of care.
- Participate in the delivery of complex care as prescribed by registered healthcare professionals.
- Plan and document patient progress on Trust information systems, maintaining professional record-keeping.
- Evaluate patient progress and discuss with senior clinicians through feedback and multi-disciplinary meetings.
- Teach and demonstrate the correct use of equipment and self-care elements to optimise patient functional ability and independence.
- Contribute to team and service development, identifying opportunities to improve service delivery.
Essential criteria
- NMC registration
- completed NA Programme
- Car Driver
- Minimum 2 years in care setting
- 2 years of health care experience

- Phlebotomy

An exciting opportunity has arisen for a qualified Nursing Associate (Band 4) to join our Rapid Response Team, (RRT) based within our Adult Physical Community Health Services, based at Goldie Leigh Abbey Wood London SE2 0AS. Bexley Adult Community Physical Health Services is part of Oxleas Foundation Trust .
RRT is an integrated health and social care team and is part of the NHSE Urgent Community Response initiative, the team provides an urgent response to patients experiencing a health or social care crisis, this is an exciting time to join the team, which has expanded to include a face-to-face response within 2 hours, falls pick up and virtual ward elements to the service.
