Role summary

Join Sirona as a Community Nurse, working within an Integrated Network Team to provide holistic and specialist care to service users in their own homes. This role offers the opportunity to develop skills through training and in-team support, focusing on person-centred interventions. You will manage a caseload and tailor care to individual health needs, including education and health promotion. The position values professional growth and offers flexible working options.

Essential criteria

  • BSc or equivalent in adult nursing & NMC registration.
  • Evidence of continuing professional development and commitment to lifelong learning.
  • Skills, Knowledge & Experience
  • Effective communication skills written, verbal and non-verbal with the ability to adapt communication for individuals with communication difficulties.
  • Able to make clinically reasoned judgements when working with a variety of conditions.
  • Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff.
  • Effective time management and caseload management.
  • Good problem solving skills.
  • Working knowledge of clinical outcomes measures, clinical audit.

Desirable criteria

  • Community work (either student or post graduate) demonstrating a broad range of clinical experience.
  • Experience of the application of health, safety and risk management policies.
  • Working with a range of professionals in health or social care.
  • Up to date knowledge of Evidence Based Practice, local and national standards.
