Role summary
This role involves providing a Nurse/Pharmacist Led Anticoagulation Service, offering expert advice and a dosing service for oral anticoagulants to outpatients. The post holder will undertake the NMP course after gaining sufficient anticoagulation competencies. Duties include discharge planning, managing an outpatient DVT service, caring for obstetric patients requiring anticoagulation, and perioperative planning for anticoagulated patients. The specialist will also run new patient clinics and provide advice on anticoagulation therapy to colleagues and clients.
Key responsibilities
- Provide a practitioner-led Anticoagulation Service, working autonomously and making independent clinical decisions.
- Run daily weekday outpatient anticoagulant clinics utilising point of care testing devices.
- Provide a telephone advisory service for patients, medical staff, and primary healthcare providers regarding anticoagulation.
- Assist in the running of daily weekday outpatient postal clinics, accurately adjusting warfarin doses according to protocol.
- Dose adjust oral anticoagulation for inpatients within SwICC and liaise with medical and nursing teams regarding related clinical problems.
- Run the Outpatient DVT clinic, facilitating outpatient management and teaching patients/carers to administer low molecular weight heparin injections.
Essential criteria
- Accredited Anticoagulant Course
- NMP or willingness to undertake as soon as sufficient anticoagulant experience has been gained (normally after 12 months)
- PACR or similar course
- Experience of anticoagulation in course of work
- Experience of caring for patients receiving anticoagulation
- In depth experience of leading a team or holding a managerial position
- In depth experience of working under own initiative and capable of critical decision making
Desirable criteria
- Completion of any anticoagulation related course
- PACR course or similar
- NMP
- Experience with anticoagulant therapy including warfarin and the DOACs
- Experience of running nurse lead clinics

Job summary
