Role summary
This is an exciting opportunity for a Registered Mental Health Nurse, Social Worker, or Occupational Therapist to join the Pennine West Home Based Treatment Team. Key responsibilities
- Undertake collaborative care planning with service users and their carers.
- Undertake risk assessment and record risks in line with trust policy.
- Undertake shift co-ordination when allocated.
- Participate in Multi-disciplinary Team (MDT) meetings and agree care plans with patients.
- Maintain knowledge of evidence-based home treatment interventions and deliver them.
- Undertake gatekeeping assessments to ensure service users are treated in the least restrictive environment.
Essential criteria
- Registered Mental Health Practitioner
- Able to understand the difference between crisis planning and contingency planning and able to formulate explicit planning in these areas
- Knowledge, understanding and experience of the care pathways for psychosis and non-psychosis and relevant interventions
- Effective communication with service users, carers and members of the multidisciplinary teams. Strong customer service and negotiation skills
- Motivational skills
- Assertiveness skills in relation to bed management
- Demonstrate the ability to work sensitively in a non-judgemental way with service users who are in crisis
- Ability to meet the travel requirements of the role
- Ability to work a 24 hour shift pattern

Job summary
An exciting opportunity has arisen for a Registered Mental Health Nurse, Social Worker or Occupational Therapist to joint the Pennine West Home Based Treatment Team. We are looking for a highly motivated, innovative and a dedicated practitioner to provide care and treatment for those service users who require support from the Home Based Treatment Team.
This dynamic and driven multi-disciplinary team offer an alternative to psychiatric hospital admission, supporting service users in acute mental health crisis in their homes, as well as supporting loved ones and carers during this period.
The team are passionate about providing person centred care, tailoring care plans to suit the needs of all individuals involved during the care spell and providing a range of psychosocial interventions during their admission.
The team are required to thoroughly risk assess situations and have access to a supportive leadership team to support such complex cases.
Staff wellbeing and development is paramount and well supported in this team.
The post holder will be supported with their professional development as part of their appraisal process, and regular 1:1 supervision is provided.
