Role summary
This is an exciting opportunity to join the Early Intervention Community Team in Birmingham, part of a large-scale Health and Social Care transformation. The role involves preventing avoidable hospital admissions, facilitating early hospital discharge, and promoting home-first rehabilitation. You will work within a multi-disciplinary team, conducting assessments and interventions to deliver high-quality care to patients in their homes and community settings. The position requires a commitment to integrated, personalised care and offers opportunities for professional development and leadership.
Key responsibilities
- Prevent avoidable hospital admissions and premature admission to long-term residential care.
- Facilitate early hospital discharge.
- Dynamically lead and promote a culture of home first rehabilitation.
- Work multi-professionally, completing assessments and interventions.
- Ensure the delivery of high quality intervention to patients within their own homes and other community settings.
- Support junior staff and students, and drive service development projects.
Essential criteria
- Registered Nurse Level 1 (Adult)
- Experience of working in a multi disciplinary team setting
- Ability to demonstrate sound knowledge and application of up to date evidence based clinical practice.
- Excellent communication analytical skills and problem solving approach, IT literate
- Awareness of complex needs of a multicultural, socially diverse population.
- Effective time management
- Ability to work under pressure
- Effective Team Member
- Leadership and interpersonal qualities.
- Needs to be independently mobile throughout the organisation
- Willing to work flexible hours including weekends and Bank Holidays as part of a rota.
- Ability to work across various sites in Birmingham
Desirable criteria
- Mentor or assessor qualification or equivalent
- Evidence of continued professional development relevant to role
- The service operates across various sites so a full driving licence and access to a vehicle is desirable.
