Job summary
An exciting opportunity has arisen for a Transition Support Worker to join the CYPS Mindworks Community Hub, North, working within the dedicated Transition Pathway . This pathway supports young people aged 17-18 as they transition out of Children's Emotional Wellbeing and Mental Health (EWMH) Services into appropriate adult provision or wider community support.
The role is designed to provide additional, personalised support to young people whose future care pathway may be unclear, ensuring they receive the right help at a critical stage in their lives. The post holder will act as a consistent and trusted point of contact, offering support over a period of up to six months.
This is a rewarding role suited to someone passionate about supporting young people through change, advocating for their needs, and delivering person-centred, compassionate care.
