We are excited to launch our new countywide Home First service on 1 September 2026 and are looking for enthusiastic and motivated Therapy Assistants to join our growing team.

Working alongside Occupational Therapists, Physiotherapists and the wider multidisciplinary team, you will support people in their own homes following hospital discharge, helping them regain confidence, maximise independence and achieve their personal goals. You will manage your own delegated caseload, delivering person-centred therapy and reablement interventions, undertaking assessments within your competency framework, and supporting people to live safely and independently at home.

This is an excellent opportunity for someone who enjoys working directly with people, thrives in a community setting and wants to be part of an innovative service focused on improving outcomes and reducing reliance on long-term care.
