Role summary
This role involves providing assessment and community support to patients who frequently attend Accident and Emergency, linking them to appropriate community services. You will manage a complex caseload of clients with mental, physical, and social needs, acting as their allocated practitioner. Responsibilities include patient assessment, care/support planning, risk assessment development, and patient document management. Key responsibilities
- To act at all times in accordance with the Nursing & Midwifery Council (NMC) code of professional Conduct or relevant professional body.
- To perform all aspects of the role in accordance with Trust policies and other national bodies.
- To participate in the assessment of care needs and the development implementation and evaluation of individual care plans.
- To provide specialist advice to other members of staff and act as a role model.
- In conjunction with the Team Manager, to take responsibility for setting, maintaining and evaluating standards of care throughout the 24-hour period.
- To liaise with all members of the multi-disciplinary team and outside agencies in the implementation of patient care.
Essential criteria
- Registered Mental Health Nurse or equivalent 1st Level Mental Health Nurse Educated at Degree level or above Evidence of continued professional development. Teaching skills. Computer Skills. Mentorship Training
- Post-registration experience to post-graduate diploma or Master's level. Evidence of clinical development/management of change. Collaborative working. Ability to provide managerial and clinical leadership. Ability to have overall 24-hour responsibility for the clinical area and MDT team. Ability to delegate. Ability to induct any staff member to the area with confidence. Sound experience in area applied for or experience in an equivalent area. Ability to tackle staff management issues, i.e. complaints and disciplinary issues. Motivated and innovative. Setting, monitoring and maintaining standards, ensuring high standards. Able to monitor staff performance and undertake PRs/action to redress poor performance. Ability to formulate and implement new ideas.

Job summary
The High-Volume User (HVU) team currently provide assessment and community support to patients who frequently attend the Accident and Emergency Department at the University Hospital North Midlands (UHNM). High intensity users of A&E departments have been identified as having a high impact on the wait times and flow of the hospital and other emergency services and health systems. If this patient group are not linked into services, A&E can become the default position. Vulnerable patients who attend A&E due to a combination of physical, psychological, and social factors often require a multidisciplinary team approach to tackle their complex and diverse issues. The HVU team will support high intensity users of A&E, linking them into the most appropriate community service in the health, mental health and/or social care or community system that is best able to meet their needs.
