Role summary
This role involves working as part of a Community Nursing Team to provide nursing care to patients registered with GPs within Midlands Partnership NHS Foundation Trust, specifically covering Cannock Villages PCN. The post holder will be responsible for assessing, planning, implementing, and evaluating patient care programmes. They will manage a defined caseload, utilise specialist practice qualifications, and employ active nurse prescribing skills. The role also includes assisting the Operational Lead with team management and care coordination, as well as leading the induction and clinical supervision of students and new team members.
Key responsibilities
- Provide nursing care without direct supervision for patients registered with GPs within Midlands Partnership NHS Foundation Trust.
- Participate in the ongoing assessment, planning, implementation and evaluation of programmes of care.
- Hold responsibility for a defined caseload of patients drawn from a GP practice population.
- Assist the Operational Lead by conducting delegated tasks instrumental in team management and care co-ordination.
- Lead in organising the induction and clinical supervision of students and new team members.
Essential criteria
- Nursing Registration with the NMC -Level 1
- Degree in Nursing or equivalent
- Mentorship/ Assessor Qualification
- Evidence of Community Experience

- Evidence of extended clinical skills and education

Job summary
