Role summary
This role is for an enthusiastic and motivated Band 6 Occupational Therapist to join a dynamic rotational service across Community Rehabilitation Hospital, Community Therapy Team, and Hospital at Home Service. You will develop specialist skills in various settings, supporting patients to maximize independence, prevent hospital admissions, and facilitate safe discharge and rehabilitation within their homes and communities. The position involves working within a supportive and innovative multidisciplinary team, providing high-quality, person-centred care.
Key responsibilities
- Manage complex caseloads across community and rehabilitation settings.
- Undertake holistic assessments and treatment planning.
- Work with multidisciplinary teams to achieve patient-centred outcomes.
- Support hospital avoidance and timely discharge planning.
- Provide supervision for junior staff, assistants, and students.
- Participate in service development, quality improvement, and clinical governance.
Essential criteria
- Appropriate professional qualification (Occupational Therapist) with full HCPC registration
- Degree level award (level 6) plus post graduate experience
- Able to demonstrate a good command of the English Language both written and verbal
- Computer Literate in both data entry and communicating via IT - emails and report writing
- Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care/community setting.
- Have a broad range of clinical skills, including assessment and treatments of a variety of conditions including neuro, orthopaedic, MSK and elderly.
- Involved in the implementation and management of change.
- Experience of working with long-term conditions and frailty.
- Demonstrates organisational skills: including the ability to make decisions and to prioritise.
- Ability to understand and interpret research findings/evidence-based care and apply to practice.
- Participation and comprehension of clinical and professional supervision.
- Knowledge of current clinical and professional issues relevant to the field of occupational therapy.
- Evidence of ability to perform multidisciplinary tasks and work across multi disciplinary boundaries.
- Experienced decision making to inform clinical reasoning approaches when managing complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
- Leadership, and communication skills, including the ability to prioritise competing tasks in a highly complex and dynamic environment.
- Ability to develop positive relationship with a range of internal and external stakeholders.
- Confidence in the application and analysis of data and audit processes to provide effective clinical governance.
- Demonstrate the required behaviour in keeping with the Trust values.
- Organisational and self management skills including those relating to own workload and those of others in an unpredictable environment.
- Good communication skills including to communicate complex, sensitive or confidential information in an appropriate manner; to liaise effectively; to understand and disseminate multifaceted information.
- Evidence of the ability to deliver learning sessions (formal or informal) to groups of professionals
- To hold a current UK driving license with business insurance cover.
Desirable criteria
- Further education training eg: external courses/masters degree or equivalent experience gained by undertaking on-going personal development and training.
- Management/Leadership Qualification/development programme
- Experience of initiating or participating in quality improvement project relating to clinical practice
- Experience working within Hospital at home or Urgent Community Response Services
- Experience in admission avoidance pathways
