Role summary
This is a full-time Senior Care Act Assessor role within a dynamic initial response team, focusing on older persons with physical disabilities. The successful candidate will work with adults over 18 with various health needs, requiring a Level 3 or 4 Diploma in Adult Care and relevant experience. The role involves working closely with multi-disciplinary teams to promote independence and improve health and wellbeing.
Essential criteria
- Level 3 Diploma in Adult Care or Lead Adult Care / Social Care Worker
- Level 4 Diploma in Adult Care or Lead Practitioners in Adult Care Level or willing to complete / achieve this if a Level 3 qualification is already held
Desirable criteria
- Work based supervisor training (Level 3)
- The ability to demonstrate a commitment to working in partnership with adults, carers, professionals and voluntary agencies, to provide a comprehensive service which meets the needs of the adults in line with current legislation and directorate, local authority & Trust policies
- Experience of working in a team, providing formal support and supervision to junior colleagues, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual's care and support plan
- Experience of working within the specialist area
- Experience of working in an NHS/Local Authority setting where MDT and specialised practice associated to social work and social care is required
- Experience in dealing with complex casework involving family members, carers, communities and other organisations
- Experience of providing informal and formal staff supervision

An exciting opportunity has arisen for a full-time Senior Care Act assessor within a well established and dynamic initial response team.
