Role summary

This role offers an opportunity to join the Integrated Community Stroke Service as an Occupational Therapist or Physiotherapist, delivering specialist stroke rehabilitation. You will conduct comprehensive assessments, provide evidence-based interventions, and support patients in achieving their rehabilitation goals, primarily in their homes. Key responsibilities include facilitating safe hospital discharges, managing stroke complications, and educating patients, families, and carers on self-management. The post requires providing support within a defined geographical area, with occasional countywide cover.

Essential criteria

  • Experience of working with multi-disciplinary and multi-agency teams, with an understanding of team roles and professional roles.
  • Experience of evaluating service delivery and monitoring effectiveness and outcomes of intervention, and of undertaking clinical audit and research

Desirable criteria

  • Experience of working in the NHS in the following clinical areas: stroke, eldercare, neurology
  • Education/ Qualifications
  • Degree or equivalent qualification in Occupational Therapy/ Physiotherapy
  • Current professional registration with the HCPC
  • Evidence of own professional development and additional specialist training in relevant area to post-graduate diploma level
  • Evidence of attending and using specialist knowledge gained from post-graduate study and courses to support practice.
  • Fieldwork educator / Clinical placement supervisor /Experience as a clinical educator of students
  • Membership of the Chartered Society of Physiotherapy (CSP) or Membership of the Royal College of Occupational Therapy (RCOT)
  • Membership of RCOT Specialist Section - Neurological Practice
  • Ability to act as an autonomous practitioner to deal with complex clinical issues
  • Specialist level assessment skills
  • The ability to deliver training both within and external to the trust/establishment

Job summary

We are excited to offer an opportunity to join the Integrated Community Stroke Service, working as an Occupational Therapist / Physiotherapist delivering specialist stroke rehabilitation. The role involves completing comprehensive assessments, delivering evidence-based interventions and supporting patients to work towards individualised rehabilitation goals, primarily within their own homes or place of residence.You will play a key role in supporting timely and safe discharges from hospital, identifying and managing stroke-related complications early, and helping to reduce avoidable readmissions.
