Role summary
This is an exciting opportunity for an experienced Registered Nurse in Learning Disabilities (RNLD) to join a Community Learning Disabilities team in North Warwickshire. The role involves managing a caseload, supervising others, completing assessments, and providing specialist nursing advice to improve the health of adults with learning disabilities. The successful candidate will work collaboratively with multi-disciplinary colleagues and act as a health facilitator, promoting the needs and rights of clients.
Key responsibilities
- Manage own caseload, completing specialist assessments and developing, implementing, and evaluating programmes of care.
- Work collaboratively with multi-disciplinary colleagues in the community setting.
- Act as a health facilitator for people with a learning disability.
- Promote the needs and rights of people with learning disabilities to other agencies and the general public.
- Identify and evaluate specific client needs through specialist assessment.
- Report any difficulties to the Team Manager as soon as possible.
Essential criteria
- Diploma / Degree holder
- Has NMC
- Teaching/mentoring qualification
- RNLD with a substantial post registration experience at Band 5 or Band 6 working with people with a learning disability
- Experience of working in multi agency teams with multi-disciplinary colleagues
- Experience of working as a community nurse
- Experience and knowledge of challenging behaviour
- Other
- Independent means of transport and UK driving licence
- Diploma / Degree holder

Job summary
An exciting opportunity has arisen for an experienced RNLD to work as part of a Community Learning Disabilities team in North Warwickshire.
The role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, Supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs and providing specialist nursing advice.
The successful candidate will need to be proactive, self-motivated and have experience of autonomous working, whilst also working under the direction of the community team manager.
The successful candidate will also be able to use current best practice to gain the best outcomes for Clients and their Carers. Will also assist in improving links with primary care services including hospitals/ G.P surgeries, provide support, information and education to clients, carers and provider's and also deliver education to people with learning disabilities around health needs both on 1:1 or group basis.
