Role summary
This Mental Health Practitioner role is within the Crisis Response and Home Treatment Team, providing urgent mental health assessments and home treatment for complex young people. Key responsibilities
- Provide high standards of evidence based client centred care to children and young people with mental health problems within the THRIVE Framework.
- Provide interventions, advice, consultation and supervision to both colleagues and external partner agencies.
- Provide high quality, evidence-based care for service users and their carers within inpatient and community services.
- Function as an associate nurse/named nurse/care coordinator to service users.
- Assist in assessment, planning and evaluation and review of care, within a recovery focused model of care.

- Appropriate professional qualification by RMN, AHP Social Worker

The team offer urgent mental health assessments and have an extensive home treatment offer supporting complex young people in the community
