Role summary
This role is for a Respiratory Specialist Nurse within the Integrated COPD Team, providing specialist evidenced-based care and support for patients with COPD in both acute hospital and home settings. The nurse will educate patients, manage their care in partnership with other healthcare professionals, and support the COPD biologics pathway. It requires excellent communication skills, a flexible approach to working patterns including weekends, and frequent travel for home visits.
Key responsibilities
- Provide specialist evidenced based care and support for patients with COPD within the acute hospital and in their homes.
- Establish contact with patients/carers to provide specialist nursing advice and practical, psychosocial support.
- Communicate to patients in a way that empowers them to make informed choices about their health and health care.
- Manage patient care to ensure patients receive care that works in partnership with other healthcare professionals, including referrals to alternative care pathways.
- Analyse, monitor and implement actions that ensure service delivery is safe and effective in line with Trust values.
- Coach patients, their families, and hospital staff facilitating a better understanding of COPD.
Essential criteria
- Registered nurse on relevant part of the NMC register
- Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience or portfolio
- Significant post registration experience / qualification / training, including experience and knowledge within relevant speciality
- Completion of all Band 5 (preceptorship) competencies
- Higher Education Institute approved course in supporting learners in practice or evidence assessed to be of equivalent experience, demonstrating support to a range of learners in practice. Provide evidence of both supporting a range of learners in practice and supporting colleagues to promote effective learning in practice
- Extensive post registration experience within specialist area.
