Role summary

Key responsibilities

Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff.Working within our Community Nursing Team you will support, assess, and treat patients with a variety of health needs, deliveringhigh quality, evidence based practice.You will holistically support patients to manage their long term conditions and care needs within the community and support patients with acute care needs to receive treatment (such as IV antibiotics) without an avoidable hospital admission.
