Role summary
This is an exciting opportunity for a highly motivated and caring Community Staff Nurse to join the North Shrewsbury locality team. The role involves providing planned nursing care to patients in their own homes or chosen place of residence, supporting timely discharge and preventing hospital admissions. The team operates a 7-day service from 8am to 6pm, and the trust offers a preceptorship programme and professional development support.
Essential criteria
- RGN
- NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post registration general experience
- Experience of working in a multi disciplinary / multi agency health/social care environment
- Experience and knowledge of basic administration systems including Microsoft office
- Experience of successful partnership working arrangements with other services
- Previous community experience
- Knowledge and Skills
- IT skills or willingness to undertake RIO Training
- Up to date with Mandatory Training
- Uses evidence based practice
- Works within own defined competencies
- Awareness of changing healthcare factors influencing change Demonstrates tact and diplomacy
- Well refined communication (verbal and written), interpersonal skills - ability to gain rapport at all levels
- Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed manner
