Role summary
This role is for an Occupational Therapist to join a friendly and supportive community multi-disciplinary team based at Solihull Hospital, focusing on falls prevention. You will manage a caseload of patients with a risk or history of falls, conducting holistic assessments in their homes and care homes. The aim is to reduce falls risk and help patients remain safely in the community. The role also involves triaging referrals, supporting junior staff, and contributing to service development.
Key responsibilities
- Provide a high standard of Occupational Therapy within the Specialist Community Falls Team.
- Undertake holistic multi-disciplinary (multi-factorial) assessments and provide relevant falls prevention interventions with patients in their own homes.
- Take on a triage and co-ordinating role within the team.
- Contribute to seven day working programmes and the development of the Falls service.
- Actively support students and new starters of all levels and disciplines.
- Be involved in new and developing initiatives within the team and wider Community Therapies.
Essential criteria
- *HCPC/RCOT Registered OT
- *Evidence of experience working within the frail or elderly patient cohort
- *Evidence of post registration CPD/ post registration qualification in an appropriate area relevant to banding
- *Demonstrates relevant and recent clinical experience enabling the post holder to assess and manage a caseload of patients with complex needs.
- *Experience of working in a multi-disciplinary team setting within a complex health care setting.
- *Experience of working in a community setting
- *Experience of comprehensive clinical assessment of patients
- *Knowledge of safeguarding issues/ procedures
- *Experience in learning from incidents /complaints and supporting changes in practice
- *Experienced in supporting the development and delivering education / training of junior staff / others
- *Experience of clinical audit / monitoring of standards / practice / benchmarking
- *Sound understanding of evidence based practice and its application
- *Knowledge of corporate and clinical governance
- *An ability to cope with a range of complex options, find solutions, analyse, compare and decide on appropriate course of action
- *Ability to communicate with a wide range of health care professionals, on a variety of complex and sensitive issues
- *Ability to work autonomously , manage / prioritise own workload across competing priorities
- *Ability to demonstrate sound clinical judgment and decision making , supported by theoretical knowledge
- *Ability to deliver evidence based care that is responsive to the needs of patients and carers
- *Ability to work in partnership with patient / carers and other health and social care professionals
- *Demonstrate excellent interpersonal skills and ability to work effectively with colleagues, patients, the public and others
- *Ability to work under pressure , manage and prioritise workload and meet deadlines
- *Be IT literate to a good standard and willingness to develop IT skills for audit and data collection and to find and appraise evidence
- *Flexible to meet the needs of the service
- *An ability to cope with moving / manoeuvring patients / equipment on a daily basis
- *An ability to frequently concentrate on patient assessments and treatments
- *An ability to concentrate and produce comprehensive documentation following an initial assessment
- *An ability to cope with occasional exposure to distressing/emotional situations
- *Be able to work within the requirements of the 7 day working scheme which will include weekend working
- *To possess a full category B car driving licence preferably clean or with no more than a maximum of 3 points
- *Able to manage a combination of mobile working and working from base location
- *To work within core service hours (8am -- 4pm) managing clinical caseload and own time effectively.

Job summary
Are you an Occupational Therapist looking to develop your knowledge and skills working in the community?
We are a friendly, versatile and supportive community multi disciplinary team based within Solihull Hospital. Our patients all have a Solihull GP, and all have a risk or history of falls, often alongside complex vulnerabilities and health and social issues. You will hold an interesting caseload of patients, working with them and their carers and relatives to reduce their risk of falls, ultimately to ensure their goals are met and they remain living safely in the community with reduced risk in the future.
Your role will include carrying out face to face holistic, multifactorial falls assessments within patients homes and care homes, before linking with the team, other community colleagues and the wider BSOL network to work together to reduce the future risk of falls for each patient. Our remit is large and we signpost and involve many different agencies to ensure that we address all identified risks as far as we can.
