This part time , role plays a vital part in supporting the delivery of safe, responsive, and high-quality care for patients across the community. The postholder will work as part of a dynamic, fast-paced service that ensures individuals receive the right care, at the right time, in the right place.
Main duties of the job
The postholder will be responsible for:
- Acting as a central point of contact for calls and referrals into service
- Coordinating care and communicating effectively with clinicians, patients, carers, and partner agencies
- Maintaining accurate and timely records using Trust clinical systems
- Supporting multidisciplinary team working to ensure seamless patient journeys
- Escalating concerns appropriately and contributing to service flow and patient safety
- Providing excellent customer service in a fast-paced and pressured environment
- Supporting service audits, data collection, and performance monitoring where required
