Key responsibilities

- Maintaining clear and accurate documentation
- Coordinating and contributing to daily board rounds and weekly MDT meetings.
- Contribute to the induction and training of new staff, including shadowing and mentoring opportunities relevant to the discharge process.
- Provide weekend cover as needed and cover other Community Hospitals when required.
Essential criteria
- Ability to travel independently between sites within the trust
- Educated to NVQ Level 3, BTEC, or equivalent qualification/experience in health or social care
- Understanding of effective discharge planning and transfer of care processes
- Experience in a health or social care setting, ideally within a hospital or community environment
- Experience of working as part of a multidisciplinary team
- Ability to communicate clearly and empathetically with patients, relatives, and professionals
- Strong organisational and prioritisation skills, able to manage multiple cases simultaneously
- Shows effective problem-solving and decision-making skills, with the ability to escalate appropriately
- Confident IT user, with ability to record, track, and report information using electronic systems
Desirable criteria
- Car owner and driver
- Evidence of ongoing professional development
- Familiarity with community care services, resources, and eligibility criteria
- Experience of discharge planning or coordination
