Role summary
This is an exciting opportunity for a Band 6 Specialist Practitioner to join the Crisis Intervention Team for Older People (CITOP) in Hull and East Riding. The team provides 24-hour crisis intervention and home treatment for older adults with functional and organic illness, including new community crisis beds. The role involves leading innovation, developing care pathways, and enhancing patient and carer experience. We are looking for motivated professionals with experience in complex needs and enduring mental illness, who can provide expert clinical advice and leadership.
Key responsibilities
- Utilise specialist knowledge of Older Adult Mental Health care to provide crisis triage, assessments, and deliver crisis interventions.
- Provide Home Treatment & Acute Community Service interventions as an alternative to hospital admission or to support timely admission/discharge.
- Collect/collate information and attend formulations for patients, working collaboratively within a Multi-Disciplinary Team.
- Develop leadership skills, provide supervision to healthcare staff, and participate in staff development through appraisal and clinical supervision.
- Work within a 24/7 service, including days, evenings, weekends, nights, and bank holidays.
Essential criteria
- Use of specialist knowledge and experience acquired in a relevant setting underpinned by theory
- A broad understanding of relevant policy, legislation drivers and their application to clinical practice and service
- Diploma or Degree in Professional qualification equivalent to RMN, Registered Social worker, OT or equivalent AHP
- Registration with the HCPC/NMC or equivalent
- Completed preceptorship programme
- Appropriate knowledge/experience of working in the relevant service area or similar
- Evidence of development of specialist practice nursing skills and able to demonstrate the impact of this on practice change/development.
- Able to demonstrate effective inter-personal/communication skills
- Work within the culture of improving working lives and working time directive
- Ability to demonstrate ethical values and attitudes within a culture of equality and diversity
Desirable criteria
- Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning
- Evidence of continuing professional development (CPD) relevant to the clinical area
- Evidence of promoting/supporting active user/carer involvement/participation
- Working knowledge of Trust policies and procedures.
- Awareness of leadership/management skills and qualities

An exciting opportunity has arisen for the role of a band 6 Specialist Practitioner to the Crisis Intervention Team for Older People (CITOP) due to the expansion of the team's current role. The CITOP team supports adults predominately aged over 65 in the Hull and East Riding area with both functional and organic illness.
The CITOP team provide 24-hour Crisis intervention and Home treatment for older adults across Hull and East Riding of Yorkshire. CITOP will be provide four Community Crisis Beds for older adults as an alternative to inpatient care and The Acute Community Service will be extended to provide a seven-day service.
Due to the development of the CITOP service we are looking for professionals who are keen to lead on innovation, developing robust care pathways and enhance the patient and carer experience.
