Role summary

This is an exciting opportunity for a proactive and highly organised Progress Chaser to join the Hospital at Home team in Stevenage. The role involves supporting patient flow, ensuring timely and coordinated care throughout their inpatient journey. You will work collaboratively with clinical colleagues, patients, carers, and partner agencies to coordinate actions, resolve delays, and support smooth, efficient discharges. Strong organisational skills and attention to detail are vital for maintaining accurate records and updating patient systems.

Key responsibilities

  • Support patient flow and ensure timely, safe, and well-coordinated care.
  • Progress patient pathways and help achieve Expected Discharge Dates.
  • Work collaboratively with clinical colleagues, patients, carers, and partner agencies.
  • Coordinate actions, resolve delays, and support smooth, efficient discharges.
  • Maintain accurate records and update patient systems such as SystmOne.
  • Support the coordination of services including therapy, nursing, pharmacy, and social care.

Essential criteria

  • Good general education including GCSE English grade C or above (or equivalent)
  • Ability to use and knowledge of Microsoft packages
  • Experience in an administrative post
  • Computer literate with good keyboard skills
  • Ability to prioritise and organise own workload
  • Ability to work unsupervised and use initiative
  • Able to communicate clearly and concisely at all levels, both verbally and in writing
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

Desirable criteria

  • NVQ Level 3 qualification in Business Administration or equivalent level of knowledge
  • Desk top publishing Knowledge of medical terminology
  • Experience of working within NHS
